PHYSICALS
Physicals must be turned in and reviewed by our Sports Medicine Staff before your son can practice.
Physical Forms are available for pick-up only at the following locations -
* SC office
* LHS office
* Athletic Office
* 24 Hour Box in front of the LHS fieldhouse
at the south end of the stadium.
CareNow is running a special for sports physicals through
July 31 for $12.00 www.carenow.com
MinuteClinic at CVS has sports physicals for $35.00 each.
